Job Summary
Partners HealthCare at Home in collaboration with PHS Population Health Management has developed a program to provide urgent care needs to patients in their homes. The Partners Mobile Observation Unit provides continued follow up urgent care for patients recently discharged home from the PHS Emergency Departments and Observation Units. The PMOU NP will provide initial urgent care services for patients identified from PHS Primary Care Practices and Specialist Offices. Upon patient referral the Nurse Practitioner will provide direct patient care in the patients homes with the goal of avoiding emergency room visits or allowing for continuity of care from hospital to home for patients who would have otherwise been admitted to the hospital(s). The program provides a unique and exciting opportunity for professionals who enjoy working in a team environment to provide the highest levels of patient centered care outcomes and comprehensive service. The ideal candidate with be a seasoned advanced practice clinician with the ability to promote the program benefits with skillful collaboration and education.
Key Activities
1.Primary management of patients admitted to the Partners Mobile Observation Unit (PMOU) in collaboration with the physician and other clinical team members to identify prevent or solve complex and/or recurring patient care issues.
2.Patients from the hospital(s) emergency department(s) primary care offices and specialist practices are identified for participation in the PMOU program through various sources across Partners Health System.
3.Performs physical examinations and evaluations for Urgent Care patients. Orders and interprets necessary diagnostic tests consultations procedures and treatments as indicated.
4.Writes comprehensive patient notes utilizing the documentation database and uploads into the electronic health record.
5.Coordinates continued followup with patients primary care providers and/or referring providers. Ensures seamless handoff of documentation current problem list comprehensive medication reconciliation and management plans with patients physicians or designee and in collaboration with the patients family.
6.Follow up with patients and families as needed to close the loop between urgent care providers and primary care providers in addition to coordinating consultations with specialists as needed.
7.Performs certain clinical administrative duties to ensure safe and smooth operation of practice.
8.Serves as a resource to providers and staff in areas of expertise.
9.Collaborates with clinical staff and other healthcare professionals to meet patients educational needs.
10.Rotating schedule requiring weekend and holiday shift coverage.
Education/Degree Requirements and Experience
Current licensure in Massachusetts as a registered nurse in the expanded role
Masters degree in Nursing required
National certification in advanced practice nursing as an Acute Care Nurse Practitioner
Two years prior experience in nurse practitioner role required
Home care experience preferred
Strong preference for experience creating clinical programs and/or working in startup situations
Demonstrated problem solving and sound judgment with complex issues
High level of service delivery to patients families and staff
Highly motivated to take initiative work independently and hold self accountable
Ability to verbally promote program highlights and benefits both formally and informally to a diverse audience
Ability to collaborate effectively with a diverse population. Listen understand and respond to concerns of patients and families while displaying compassion and advocacy for patient needs.
Exceptional organizational skills with ability to prioritize effectively and complete tasks.
