Your role and responsibilities
As a key member of the Home First Service team you will play a vital role in supporting our locality by completing care visits and assisting with Short Term Reablement Plans (STEP). Your contributions to assessment and reablement will be essential in helping our customers achieve their goals. You will mentor and direct new staff aiding their development and ensuring they are wellprepared for their roles. Liaising with multidisciplinary teams you will support your customers and work closely with team colleagues and the Home First team Leader to handle tasks such as medication observations addressing issues and training new staff in basic skills.
As a risk assessor you will work with customers to identify any risks ensuring safe care and support. You will be the senior point of contact for Reablement Support Workers providing initial advice and guidance. With Trusted Assessor training you will be able to order small pieces of equipment to help customers maintain their independence. Your involvement in frontline care delivery will include assisting customers with dignified and intimate personal care needs when necessary.
Engaging with customers family carers and others in their network you will handle more complex care packages with the right training and support to ensure confidentiality and safety. You will promote alternative ways to meet and maintain customers longterm needs advocating for a range of services that help people live independently and safely at home. Collaborating with assessment staff you will monitor customers progress towards their goals and outcomes completing records for the ongoing review process. Additionally you will have opportunities for development into specialisms and become a champion in your team for various skillsets.

The team
Home First provides a short term assessment and reablement care and support service to people living in their own homes. This may be following a stay in hospital an illness or crisis at home or help to avoid an admission into hospital. You will be working closely with our colleagues from the Integrated Neighbourhood Teams (INTs) which include Social Work Teams Community Health Independent Providers GPs the local councils voluntary services as well as many others. You will also work closely with Safeguarding colleagues.
