Job Description
Must be licensed to practice in the Commonwealth of Massachusetts.
Responsible for reviewing and assessing the quality of pharmacotherapy for patients in assigned coverage areas. Responsible for the optimization of drug therapy by identification resolution and prevention of drug related problems and improving therapeutic outcomes.
Also responsible for processing medication orders entered in providerorderentry (and written) by the prescribers for promptly and accurately providing medications for administration to patients and for supplying pertinent timely and accurate information to health care workers and patients for the safe and effective use of medications.
Work independently without direct supervision within the limits of established pharmacy practice and within the scope of departmental policies and procedures as a member of the patient care team. Represent the Pharmacy on key programs including serving as a liaison serving on committees as appropriate recommending policies and procedures and reviewing drug and supply requirements for that program.
