Job Description
- Self directed clinical exercise physiologist with strong clinical skills and experience in Cardiac Rehabilitation.
- Demonstrated expertise in secondary prevention setting multidisciplinary team function and case management experience preferred.
Assess plan manage and evaluate care for patients in Phase 2 and 3 Cardiac Rehabilitation Program under guidance of a RN
Responsible for equipment maintenance and assist with program referral management
Participate in management of participants in other clinical programs at the CV Disease Prevention Center
Exercise prescription
Patient care planning and evaluation in a case management model
Independent mature
Familiarity with database function/data management

- BS/BA Exercise Science/Kinesiology/Exercise Physiology Required
- MS preferred
- Minimum 5 years experience exercise physiology with Cardiac Rehabilitation concentration
- ACSM exercise specialist certification preferred
