The Division of Thoracic Surgery believes in the team approach to medical care. In collaborating with Physicians Physician Assistant will be involved in: Coordination of preoperative assessments; Coordination of treatment and assistance in management of preoperative assessments; Coordination of treatment and assistance in management of postoperative care and discharge planning.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
During the preoperative evaluation the P.A. will:
Assist in preoperative evaluation and education. Perform initial interview and assessment of patient. Perform history and physical examination.
Review diagnostic data May obtain preoperative surgical consents.
Prepare preoperative data sheets.
Present preoperative patients at team meetings.
Monitor patients undergoing neoadjuvant protocols.
Identify patients requiring preoperative nutritional assessments.
Liaison with the PreAdmission Testing Center (PATC) surgical scheduling areas and nursing units.
Communicate any abnormal findings to chief resident for inpatient and to attending surgeon for outpatients.
Track and obtain radiographic studies required preoperatively for patients.
During the postoperative hospitalization and discharge the P.A. will:
1. Attend daily rounds with the Thoracic team and round independently.
2. Coordinate transfers from the Thoracic Intermediate Care Unit (TICU)
3. Assist with and perform independent diagnostic procedures: insertion of chest tubesremoval of chest tubes bronchoscopies wound care removal of drains interpleural chemotherapy etc. as acceptable via the guidelines of the Division of Thoracic Surgery and Brigham and Womens Hospital.
4. Obtain consultations from other services.
5. Coordinate patient discharge along with nursing staff to include special needs at home medications proper referrals (VNA PT etc) and coordinate proper followup appointments as deemed appropriate by chief resident and team.
6. Assist with Day After Discharge Program (DAD) where upon each patient is called at home 24 hours after discharge to assure the transition back to the home or facility was without complications. This will also include same day call to the referring physician alerting him/her that patient has been discharged that day.
7. Coordinate followup at outside facilities and assist in communication with other facilities in question of patient care.
8. Admissions to the floor (H&P orders)
9. Renew medications transfer orders and general orders under supervision of physician.
10. Assist TICU Director in the development of protocols and procedures for P.A.s residents and nurses.
11. Provide daily resource for staff in the TICU and other units.
12. Develop and implement routine instructions for patients discharge.
REPORTING RELATIONSHIPS:
Directly reports to the Chief of the Division of Thoracic Surgery. Consults with the Administrative Director of Thoracic Surgery on all administrative matters. Interacts extensively with physicians from BWH ambulatory services operating room inpatient and admitting staff.

QUALIFICATIONS:
The successful candidate will be a graduate of an accredited program for Physician Assistants hold a Masters degree and be eligible for Massachusettss licensure or presently licensed by the Commonwealth of Massachusetts Board of Registration for Physician Assistants. The individual should have 3 years of relevant clinical experience. The Division is an extremely busy service within the Brigham and Womens Hospital. The candidate must be mature willing to learn organized and able to accept direction. Ability to cover a rotating shift including evening weekend and holiday call/coverage.
WORKING CONDITIONS:
The work may require the lifting of heavy objects and patients as well as exposure to potentially infective material. Work requires contact with needled and other sharp instruments electriccautery devices monitoring devices and other stateoftheart equipment. Prolonged standing can be expected. The hospital is approximately mile long and excessive walking to and from the patient care units can be expected. This position has a rotating shift which includes evenings weekends and holidays.
SUPERVISORY RESPONSIBILITY:
Supervises medical students rotating through the service. Teaching other health care professionals the management of thoracic surgical patients.
DRESS CODE:
Professional dress code required. No jeans sneakers shorts or teeshirts.
