Position Summary: Become a vital member of the NEW HomeBased Crisis Intervention (HBCI) Team in Wayne County NY. Make a meaningful impact on youth aged 520 with behavioral health needs through shortterm therapeutic interventions that support families in their own homes and communities. We celebrate diversity and foster an inclusive environment where every family feels valued.
Responsibilities:
- Provide supportive clinical services to 23 families at a time conducting regular to daily meetings with each child/youth and family.
- Deliver crisis intervention prevention and resolution strategies.
- Engage in frequent outreach to clients and their families.
- Conduct strengthbased assessments to understand clients social determinants of health mental wellbeing resilience to trauma potential risks and substance use factors.
- Create personalized service plans tailored to individual family needs.
- Design and implement effective service delivery strategies.
- Collaborate with partner service providers to coordinate comprehensive treatment plans.
- Maintain thorough case management records to ensure effective service delivery.
Requirements
Education:
- Bachelors in human services psychology social work or other related field required
- Masters in Social Work Psychology SocialWelfare Human Services or other related fields preferred
- Licensed mental health provider or unlicensed Masters preferred
Experience:
- 2 years experience in case management community based mental health and other related responsibilities required
- Drivers License and ability to maintain insurability throughout employment required.
