Job Description

Position: Transition Teacher 1822 Program)School Year Location: Tiverton Public Schools Tiverton RI Job Type: FullTimeAbout the Position:Tiverton Public Schools is seeking a dedicated and dynamic Transition Teacher to support students aged 1822 with developmental disabilities as they transition from high school to adulthood. This program focuses on developing independent living skills vocational training community engagement and postsecondary readiness. The ideal candidate will be passionate about fostering student independence and preparing them for successful adult lives.Responsibilities:Develop and implement individualized transition plans aligned with student goals and IEPs.Provide instruction in independent living skills job readiness functional academics and socialemotional development.Collaborate with families community organizations and local businesses to create workbased learning and vocational opportunities.Support students in communitybased experiences including job sites volunteer work and public transportation training.Utilize evidencebased instructional strategies and assessments to measure student progress.Work closely with special education staff related service providers and outside agencies to ensure a coordinated approach to transition planning.Maintain accurate records progress reports and documentation in compliance with district and state requirements.Foster a positive inclusive and studentcentered learning environment.
