Description

Family Stabilization Services Coordinator

Compensation: $46000$48000

This is a communitybased position.

Family Stabilization Service Coordinators with National Youth Advocate Program work in the community to make a critical impact in the lives of youth and their families. Under the general supervision of the Clinical Supervisor or designee this position works closely with youth family foster parents and community partners providing advocacy and support toward the ultimate goal of living safely and successfully in the community.

NYAP is participating agency in the Great Minds Fellowship and Great Minds Workforce Commitment Incentive Programs.

Responsibilities

Must have flexible availability including evenings and oncall rotations.

Provides in home supportive services transportation parenting education counseling community resource referral advocacy and other social service interventions to the youth and family.

Travels daily to provide school based and communitybased services to and on behalf of youth and families

Coordinates and monitors services for the youth and family in the community including mental health medical educational psychological vocational and social services.

Establishes and maintains strong relationship with assigned youth and families.

Provides safety assessment crisis response and behavioral stabilization services as required to assure safety and stability for youth served.

Minimum Qualifications

Bachelors Degree in Social Work or comparable Human Services field from an accredited institution.

2 years of work experience working in direct service with youth and families strongly preferred.

Proficient use of desktop and laptop computers smart phones and tablets printers fax machines and photocopiers as well as software including word processing spreadsheet and database programs.

Driving and Vehicle Requirements

  • Valid drivers license
  • Reliable personal transportation
  • Good driving record
  • Minimum automobile insurance coverage of $100000/$300000 bodily injury liability
