Case Manager II


People Incorporated Mental Health Care Services is seeking a Case Manager II to join our Childrens Targeted Case Management team. This program helps individuals under 18 years of age gain access to medical social educational vocational and other necessary services connected to the persons mental health needs. Eligible children must have a mental health diagnosis. This is a fulltime hybrid position located in the Twin Cities Minnesota.


Who We Serve

  • Children age 517 years of age
  • Children served are struggling with functioning at home in school or in the community
    due to a serious mental health diagnosis

Our Services:

  • Assessing the childs needs goals and the impact of mental illness on their everyday life
  • Referring and linking to resources supports and services including: individual and family therapists primary care physicians individual and/or family Childrens Therapeutic Services and Supports (CTSS) skills workers individualized education plan (IEP) evaluation health insurance and psychiatrists
  • Monitoring the effectiveness of the resources supports and services being utilized
  • Discussing the progress made toward goals and recovery

Hours: FullTime MondayFriday typical business hours some late afternoon and early evenings as needed


Location: 5555 Boone Ave New Hope MN 55428 Hybrid


Hiring Range: $24.00 $25.35/ Hourly depending on qualifications


Hiring Bonus: $1000 available to external candidates

Main Job Duties:

  • Must be able to have physical presence in the Twin Cities Minnesota.
  • Act as an internal resource to other Case Managers providing onsite counsel and establishing best practices on program expectations.
  • Develop and maintain subject matter expertise in a particular area that allows for continued client care and effective sharing of information to others so they may implement.
  • Provide temporary case management services during program transitions to ensure that program service levels are maintained.
  • Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
  • Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
  • Maintain regular and active contact with clients ensuring services sought are utilized and partnering with the client to determine effectiveness of the plan and next steps.
  • Provide general knowledge and guidance on assisting clients in the multiple phases of applying for and receiving services and benefits.
Requirements

Qualifications for all Case Managers:


To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge skill and/or ability required.   • Passing all background and requirements for facility and position; and
  • The ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
  • Valid drivers license excellent driving record and current auto insurance

Required Qualifications for Case Manager II:

  • Bachelors Degree in behavioral science or related field

AND

  • One Year 2000 hours) of fulltime providing mental health services to children and their families under supervision of a licensed mental health professional

OR

  • Masters degree in behavioral science or related field

Preferred Qualifications:

  • Bilingual (English/ Spanish)
  • Knowledge of Hennepin County Human Services Systems
  • Knowledge of adolescents mental and chemical health issues and awareness of communitybased mental and chemical health resources
  • Experience with children/ adolescents and families
  • Ability to forge a mutually respectful partnership with persons served and their families
  • Knowledge of local community resources and how to use those resources for the benefit of children and their families
