Job Description

We are looking for a Care Manager to enhance the quality of client experience and be accountable for the client and/or family satisfaction with the interaction with our company. Our team is passionate about making a positive impact in the lives of those we serve and we are looking for a compassionate organized and dedicated Care Manager to join our team. Care Manager duties include assessing planning implementing monitoring and evaluating actions required to meet the clients health and human services needed.

Key Responsibilities:

  • Conduct assessments to understand clients needs strengths and challenges.
  • Develop and implement individualized care plans that address client needs and goals.
  • Coordinate services with community resources and service providers.
  • Monitor client progress and adjust care plans as necessary.
  • Advocate for clients to ensure they receive the necessary services and support.
  • Maintain accurate and uptodate client records.
  • Adhere to infection control and safety procedures.
  • Provide emotional support and guidance to clients and their families.
  • Assure the plan of care includes the clients functional limitations and needs
  • Ensure compliance with all relevant laws regulations and standards.
  • Participate in team meetings and case reviews.

Qualifications:

  • Previous experience in case management or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Excellent organizational and timemanagement skills.
  • Knowledge of community resources and services.
  • Compassionate empathetic and clientfocused mindset.
  • Ability to handle sensitive and confidential information with professionalism.
