Job Description
RN Case Manager
San Fernando Valley California
The Case Management process facilitates communication and Care Coordination along a continuum through effective transitional care management and Utilization Management. The Case Manager is a collaborative practice including patients caregivers nurses social workers physicians payers support staff other practitioners and the community. RN Care Manager works an 8hour Variable shifts and must have availability to work 8am4:30pm or 4:30pm1am with weekend rotations.
Qualifications:
- Registered Nurse required
- Bachelors Degree in Nursing Or equivalent education/experience within 3 years of hire
- California Fire and Life Safety Card National Organization within 30 days of hire
- California Registered Nurse License
- Case Management experience in an Acute Hospital setting
- Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Manager
- TIP candidates must have experience in same type of nursing unit in which the Case Management position is available
Preferred Qualifications:
- Masters Degree in Nursing or Healthcare related field
- Upon hire: Certification in Case Management (ACM or CCM)
