Job DescriptionAmesbury High School is seeking a dynamic special educator to work with 1822 year old special education students in a transitionbased program serving as their special education liaison/case manager. The ideal candidate will work with young adults in the Amesbury community to establish work opportunities develop relationships with supporting adult agencies and continue ongoing education in vocational and life skills. The goal of the program is to support transition into adult life in the community. Job Responsibilities and Functions:Serves as Special Education Liaison/Case Manager and Transition Teacher for students on IEPs aged 1822Coordinates all phases of the transition planning process in collaboration with the Special Education Facilitator and the Director of Student Services as neededAttends/participates in all IEP meetingsDevelops transitional IEPs and transition plansWrites and implements developmentally appropriate transitional goalsCollects and submits data relevant to IEP goals and reports to students families and staff Conducts transition assessments as neededCollaborates with other special education staff and related services providers (OT PT Speech etc. in support of students Collaborates with school counselors to support counseling goals and postsecondary planningDevelops and implements learning modules which facilitate the generalization of transitional goals including selfdetermination selfadvocacy selfawareness exploring postsecondary options career awareness and interviewing skillsDevelops student/staffing schedules for community outingsCollaborates with regional postgraduate programs Consults and confers with IEP Team regarding appropriate transitional goalsWorks directly with students within a vocational and transitional counseling frameworkTransports students to job/training sites using school vans and/or travels with students on public transportationFacilitates access to and utilization of community resources (e.g.public transportation banks libraries career centers grocery stores laundromats etc.)Appropriately fades supports to students in an effort to promote independenceWorks with MassAbility (and appropriate EOHHS state agencies such as DDS MCB DMH or DPH) and other necessary agencies to assist parents and students with the postsecondary enrollment process and transition to adulthood and adult servicesSeeks additional resources that will enhance services provided to postsecondary studentsMakes contact and creates partnerships within the school and community for the purpose of developing job shadowing and work training opportunitiesProvides job coaching within the school and communityDevelops and pilots new curriculum to enhance student abilities in acquiringage appropriate independent skillsAssesses and monitors conduct of transition students that are within his/her direct chargeLead interagency transition meetingsMaintains records of appropriate documentation during the transition process.Attends administrative meetings and work sessions as required and attends professional seminars conferences and workshops as required by the positionCollaborates with paraprofessionals supporting students in the classroom and communityDisseminate transition information and provide professional development to teachers/administrators as neededSupports the maintenance of records for students with special needs to document service deliveryPerforms any other duties assigned by the Building Principal and/or Director of Student Services
