Job Description

Responsible for the evaluation clinical assessment treatment plan performance of SpeechLanguage interventions and treatment plan progression for physicianreferred patients that demonstrate speech language hearing and oral myofuctional disorders. Responsibilities also include consulting with physicians and the health care team regarding patient care participating in team and patient care participating in team and patient/family conferences educating physicians and associated to speechlanguage interventions to insure continuity of care and completing all required paper work such as clinical records care plans and charge sheets.

MINIMUM QUALIFICATIONS:

EDUCATION CERTIFICATION AND/OR LICENSURE:

1.Masters Degree from a school of Speech Language pathology.

2.Current West Virginia SLP Therapy Licensure or applicable state licensure where services will be performed.

3. CPR certification within 30 days of hire date.

4. Valid drivers license.

PREFERRED QUALIFICATIONS:

EDUCATION CERTIFICATION AND/OR LICENSURE:

1.Member of (ASLHA) American Speech Language Hearing Association.

2.Must possess valid WV drivers license and must have automobiles insurance of $/on his/her vehicle.

1.Performs evaluation procedures according to policy/procedures according to policy/procedure or standard of care including but not limited to: dysphagia evaluation voice evaluation communication evaluation cognitive assessment language evaluation hearing screens.

2.Safely and effectively delivers and analyzes speechlanguage interventions as per clinical policies and procedures.

3.Implements home programs meets patient/family educational needs as per policy and procedure.

4.Promptly and thoroughly completes required clinical documentation including evaluations progress reports discharge summaries and care plans.

5.Instructs and delegates restorative speech language programs to the nursing staff to insure continuum of care as per policy and procedure.

6.Calls patients timely and follows visit pattern.

7.Performs CPR when directed by patients/appropriate representatives wishes.

8.Demonstrates bag technique per policy and procedure.

9.Apply and maintain equipment according to the Policy and Procedure Manual.

10.Follows supply requisition systems as per policy and procedure.

11.Evaluates the effectiveness of the plan on a regular basis.

12.Coordinates the care team in order to facilitate a change in the level of care in compliance with the change in level of care standard.

8. The majority of work is performed in the patients home and may require extensive travel.

SKILLS AND ABILITIES:

1. Must have strong communication skills written and verbal.

2. Must demonstrate good initiative and sound judgment in the evaluation and delivery of speechlanguage services.

3. Must be able to perform the physical task of written communication.

4. Must have good time management skills.

5.Demonstrates knowledge of and compliance with safety policies and procedures.
