Job Description Summary:
The Home Health Aide is a paraprofessional member of the home care team who works under the supervision of a Registered Nurse or Therapist and performs various personal care services as necessary to meet the patients needs. The Home Health Aide is responsible for observing patients reporting these observations and documenting observations and care performed. The Home Health Aide will be assigned in a manner that promotes quality continuity and safety of a patients care. Respects and promotes the mission vision and values of VNA Health.

Job Duties:
Responsibilities of the Home Health Aide include but are not limited to the following:
  1. Providing personal care including:
    1. Baths
    2. Back rubs
    3. Oral hygiene
    4. Shampoos
    5. Changing bed linen
    6. Assisting patients with dressing and undressing
    7. Skin care to prevent breakdown
    8. Assisting the patient with toileting activities
    9. Keeping patients living area clean and orderly as appropriate
  2. Planning and preparing nutritious meals.
  3. Assisting in feeding the patient if necessary.
  4. Taking and recording oral rectal and axillary temperatures pulse respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency).
  5. Meets the average daily productivity standards as set by the agency.
  6. Knowledge of state federal local and accreditation regulations pertaining to the delivery of home health services.
  7. Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
  8. Assisting patient in the selfadministration of medication.
  9. Doing patients laundry as appropriate.
  10. Meeting safety needs of patients and using equipment safely and properly (foot stools side rails etc..
  11. Reporting on patients condition and significant changes to the assigned nurse.
  12. Adhering to the Organizations documentation and care procedures and standards of personal and professional conduct.
  13. Attends monthly meetings and inservices as required.

All clinical staff are expected to participate as needed in variable shifts including weekend rotation per staffing needs and departmental holiday rotation.
Qualifications:
  1. Current and active California Home Health Aide Certification.
  2. Current CPR certification.
  3. Licensed driver with insured automobile in accordance with state and/or organization requirements.
  4. Ability to read and follow written instructions and document care given.
  5. Selfdirecting with the ability to work with little direct supervision.
  6. Empathy for the needs of the ill injured frail and the impaired.
Physical Requirements:
  1. Ability to lift 50 lbs.
  2. Ability to push/pull 200 lbs.
  3. Ability to transfer 170 pounds.
  4. Ability to stoop bend and squat.
  5. Ability to walk up one flight of stairs.
  6. Ability to walk one mile.
  7. Ability to sit for one hour minimum.
  8. Visual and audio acuity.
