Responsibilities:
- Provide direct patient care to residents including administering medications wound care and other medical treatments
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Conduct resident assessments identify care needs and make recommendations for modifications to care plans as needed
- Maintain accurate and timely documentation of resident care
- Communicate effectively with residents families and healthcare providers to ensure seamless coordination of care
Requirements:
Current LN license in good standing
1 year of experience in a skilled nursing facility or similar healthcare setting
Basic Life Support (BLS) certification
Ability to work flexible hours including days evenings and weekends
