Job Description

Helping Hands is looking for a PRN Certified Nursing Assistant to be part of our Home Health division.

Overview

Certified Nursing Assistant will assist the consumer with the tasks of daily living as outlined in a written care plan.

  1. BASIC PURPOSE

According to prescribed policies and procedures of the organization including all applicable state federal and accreditation regulations and under the supervision of the RN the Aide/Homemaker is a nonprofessional employee trained to provide personal care and related services to patients in this agency. They assist the patient in his/her activities of daily living (ADLs).

  1. PRIMARY RESPONSIBILITIES
  1. Responsible for performing and documenting all duties assigned by the Clinical Director or Hospice Registered Nurse (RN).
  2. Observes records and reports the patients care symptoms emotional and physical status.
  3. Provides personal care including but not limited to:
    1. Bathing
    2. Grooming and shaving
    3. Back rubs
    4. Oral hygiene
    5. Hair care
    6. Changing bed linens/make the bed
    7. Assisting patients with dressing and undressing
    8. Skin care
    9. Foot care
    10. Ear care (Qtips cannot be used)
    11. Nail care (nail cutting or filing is not to be done)
    12. Assisting the patient with toileting activities
    13. Keeping patients living area orderly as appropriate
  4. Assists in feeding the patient as needed.
  5. Documents appropriately according to the patients plan of care and submit all documentation per agency policy.
  6. Keeps a record ofaide/homemaker plan of care for each patient assigned.
  7. Watches observes and reports any physical and gross behavioral changes in the patients condition or family situation to the RN.
  8. Follows the visit schedule and indicates changes in schedule and reports those to the agency as appropriate.
  9. Promptly contacts patients and the agency supervisor whenever unable to visit a patient as scheduled and will give full report to covering aides concerning patient care visit times etc.
  10. Communicates appropriately with the patient/family and agency personnel.
  11. Obtains vital signs if needed or ordered records and notifies the primary RN of deviations according to standard practice.
  12. Assists in ambulation transfers and exercise (which has been taught by a health professional of the agency) according to the plan of care.
  13. Performs range of motion and other simple procedures within theAide/Homemaker scope of care as needed.
  14. Incidental household services which are essential to the patient health care.
  15. Does patient laundry as appropriate.
  16. Meets safety needs of patients.
  17. Understands and applies basic infection control principles and procedures. Contacts supervisor with any concerns related to infection control.
  18. Follows the patients plan of care as instructed by the agencys healthcare professional.
  19. Ensures all care is provided with respect and dignity to patients and families; reports all complaints made by patients or families to the appropriate supervisors; reports all allegations of abuse misappropriation of patient property and any other compliance issues immediately.
  20. Performs other duties as assigned.

Quality Assessment/Performance Improvement (QAPI)

  1. Participates in the performance improvement process by finding opportunities for continuous monitoring and identification of patient outcomes intra/interdepartmental and environmental concerns.
  2. Displays a willingness to utilize problem solving techniques to insure appropriate interventions and resolutions.
  3. Participates in clinical record review and assures proper maintenance of records per agency guidelines.

Secondary Duties

    1. Participates in staff meetings department meetings Hospice IDT meetings inservices committees and other related activities as required.
    2. Attends required inservices and meetings as identified by the agency must have twelve 12 hours of inservices a year.
  1. PHYSICAL REQUIREMENTS See Attached Physical Requirements Form.
        1. Visual/hearing ability must be sufficient to comprehend written/verbal communication.
        2. Must have excellent verbal and written communication comprehensive computer and interpersonal skills.
        3. Able to effectively manage stress.
        4. Must have the emotional maturity necessary to establish and maintain a good rapport with patients and their family members
        5. Able to work in a variety of settings and conditions in patients homes and facilities.
        6. Must be able to meet the physical demands of the position.
  1. EDUCATION AND EXPERIENCE
    1. Education:
    1. High school diploma or equivalent.
    2. Completion of an approved hospice aide training program as outlined under the Hospice Conditions of Participation 418.76: Hospice Aide/Homemaker Services; and proof of certification as a Hospice Aide from the program.
      OR

Completion of a nurse aide training and competency evaluation program approved by the State as meeting the requirements under Medicare 483.151 through
483.154 and is currently listed in good standing on the State nurse aide registry

OR

Completion of a state approved training program and certification as a home health aide and proof of certification.

    1. Must demonstrate skills and have a passing score on a written competency test in accordance with 42 C.F.R. Section 484.36 prior to working independently with a patient.

    1. Experience:
    1. Minimum age requirement of 18.
    2. Minimum of one 1 year of direct patient care experience in an institutional or home care setting required with community health/hospice experience preferred.
    3. Must demonstrate the ability to be able to work within a multidisciplinary team setting.
    4. Understanding of Hospice philosophy and needs of terminally ill.
  1. Licensure Registration and/or Certification:
    1. Valid drivers license and reliable transportation that is insured in accordance with Pennsylvania requirements.
    2. Satisfactory references from employers and/or professional peers.
    3. Satisfactory criminal background and national sex offender checks.
