As an Advanced Practice Provider youll play an integral role in our ambulatory care team by providing highquality costeffective care to patients.
We are seeking a Nurse PractitionerOR Physician Assistant to:
- Conduct psychiatric assessment of adult patients with acute and chronic mental illness in an outpatient setting. This includes the assessment of the patients psychosocial cognitive developmental and medical status.
- Complete assessment diagnosis treatment management education health promotion and care coordination for patients with acute and chronic health needs
- Guide treatment planning which may include referral for individual therapy group therapy interventional treatments for depression IOP or PHP level of care
Education:
If Nurse Practitioner:Masters or Doctorates degree from accredited Nursing Program.
If Physician Assistant:Bachelors or Masters degree from accredited PA Program.
Licenses and Certifications:
Minimum State of Wisconsin NP or PA licensure
If Nurse Practitioner*:
- Active RN and APRN certifications and licenses including prescriptive authority in state(s) of practice AND
- Active board certification as a NP in area of clinical practice. These may include the following:PsychiatricMental Health.
If Physician Assistant*:
- Active PA license including prescriptive authority in state(s) of practice AND
- Active PA board certification
All Nurse Practitioner and Physician Assistant applicants must have*: Active DEA license or ability to apply for such license prior to or upon hire
