Job Description
We are a small employment agency based in Albany New York.
Rooted with community and better health initiatives and consistent with the highest standards of
quality care our client seeks to be the premier provider of health services in the greater
metropolitan area ensuring access to health supportive and communitybased services across the
continuum of need. Their range of health services include home care hospice and palliative care for
adults and children rehabilitation and nursing care and the researchbased institution for
innovation in palliative care. They also offer health plans for Medicare and dualeligible individuals.
Their ability to serve atrisk members of the community is supported by the efforts of their nonfor
profit foundation which raises funds to help support their quality programs and services not
reimbursed by insurance or government programs. In addition the generosity of grateful families
private and corporate donors and grants as well as their own employees also support their
specialized services.
Responsibilities:
Supports the Hospice Nurse Manager position by providing leadership guidance to the professional
staff on a daytoday basis to ensure the provision of Hospice and Palliative Care services are
consistent with the philosophy goals and policies of the our client. Mentors and provides
leadership direction to the professional staff as directed by the Hospice Nurse Manager.
The schedule includes functioning in a backup On Call capacity 23 times per month and 23
holidays per year.

Qualifications:
Graduate from an accredited School of Nursing BSN or BS degree preferred. Minimum of five years
of Hospice nursing experience. Supervisory or leadership experience preferred. Word Outlook and
Windows experience required. New York State RN licensure NYS valid license preferred. National
Board Certification in Hospice & Palliative Care (CHPN) preferred or obtains certification within one
year of hire. Ability to effectively supervise and mentor Hospice nursing staff.
Shift: Monday Friday 9:00 am 5:00 pm
Salary: $106000 $125000
