PATH is seeking candidates passionate about helping others make a positive change in their lives to join our ICMS Department as a ReEntry Case Manager working out of our Los Angeles office on Madison Avenue.

ABOUT PATH
Since its foundation in 1984 PATH (People Assisting the Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California connecting our clients to a comprehensive continuum of homelessness prevention street outreach employment preparation and placement assistance individualized case management supportive services interim housing and permanent supportive housing.

ABOUT OUR REENTRY ICMS PROGRAM
PATHs ReEntry Intensive Case Management Services (RICMS) Program provides services to individuals reentering society following criminal justice involvement. Reentry intensive case management services include but are not limited to outreach/ engagement case management assessment and service planning interim and permanent housing application housing location coordinating needed health services and addressing all psychosocial service needs to stabilize clients in housing reduce functional health and mental health barriers promote recovery community integration and socialization.

ABOUT THE CASE MANAGER
Case Mangers work as part of a multidisciplinary team comprised of other case managers housing providers and subsidy funders and community providers including JCOD. Within the RICMS Program Case Managers are responsible for providing fieldbased directclient ICMS services designed to stabilize clients in their return to society.

Position Responsibilities include:

  • Provide fieldbased and mobile case management services in the areas of but not limited to: Independent living skills housing stabilization money management community integration employment linkage benefits establishment linkage to community providers for substance abuse primary and mental health care and all other services needed to assist clients in reaching their treatment plan goals.
  • Provide strengthsbased case management and service coordination services designed to assist clients in obtaining and maintaining stable housing.
  • Provide necessary and indicated direct care and referral services to an assigned caseload.
  • Develop individualized treatment plan in collaboration with client addressing shortterm and longterm goals.
  • Provide ongoing case management support to assess progress and ensure treatment plan outcomes are met or changed as needed.
  • Conduct crisis and risk assessments in consultation with supervisor/team or LCSW.
  • Provide crisis intervention services focused on enhancing the clients ability to independently problem solve utilize effective coping skills and manage and selfcoordinate own care.

WHAT YOU BRING
Were looking for candidates that:

  • Maintain and execute confidential information according to HIPAA standards
  • Demonstrate knowledge of or experience with evidencebased case management techniques including critical time intervention trauma informed care harm reduction strategies crisis intervention techniques and motivational interviewing
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement and are highly motivated selfstarting and proactive
  • Are excellent at communicating whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a highpressure environment

PREFERRED QUALIFICATIONS

The following education and professional experience is preferred:

  • Two 2 years of experience in healthcare case management homeless services or working with vulnerable
    populations or a related field.

MINIMUM QUALIFICATIONS

  • All levels of education and experience are welcome

MANDATORY REQUIREMENTS
For this role the Case Manager must:

  • Be able and willing to work flexible hours which may include evenings or weekends
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID19 vaccination
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test

DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role all candidates must:

  • Have reliable transportation
  • A valid drivers license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATHs insurance coverage
