You will be updated with latest job alerts via emailJOB TITLE: Wraparound/Visitation Specialist
JOB PURPOSE: The CommunityBased Wraparound Specialist provides comprehensive support to families by overseeing and monitoring family visits delivering traumainformed care and facilitating positive family dynamics. This role requires a compassionate professional with expertise in case management parent coaching and the ability to deliver effective parenting classes. The specialist will work directly with families in their homes and community settings to enhance familial relationships and ensure a safe supportive environment for children and caregivers.
ESSENTIAL FUNCTIONS:
Family Visits
Monitor and facilitate family visits to promote positive interactions and ensure the safety and wellbeing of all parties involved.
Conduct inhome and communitybased visits to assess family dynamics and provide direct support.
Case Management
Develop and maintain comprehensive case plans tailored to the individual needs of each family focusing on strengthsbased solutions.
Document family progress case notes and all interactions accurately and promptly in accordance with agency protocols.
TraumaInformed Support
Apply traumainformed care practices / Sanctuary model to address and mitigate emotional and psychological challenges experienced by families.
Provide crisis intervention and stabilization support when necessary.
Parenting Education
Deliver parenting classes and personal coaching sessions to enhance parenting skills and foster healthy family environments.
Identify and leverage community resources to support families diverse needs.
Collaboration & Advocacy
Work collaboratively with internal teams external agencies and community partners to coordinate comprehensive services for families.
Advocate for families within educational social services and healthcare systems as needed.
Professional Development
Participate in ongoing training and professional development opportunities to stay current with best practices in family support traumainformed care and case management.
OTHER FUNCTIONS: Attends staff managers and departmental meetings. Contributes to departmental and organizational decisionmaking through active participation in the process. This position requires occasional evening or weekend work. Ability to travel for meetings between site locations conferences and events as needed. Works effectively and positively with coworkers stakeholders and volunteers. Other duties as assigned by director of family and community programs and executive director.
QUALIFICATIONS:
: Bachelors degree in social work counseling psychology human services or 5 years work experience equivalent in a related field. Minimum of 2 years of experience in case management or providing direct support to families in a social service setting. Proven experience with traumainformed care practices. Strong interpersonal and communication skills with the ability to build trust and rapport with diverse populations. Demonstrated ability to coach and support family members in developing parenting skills. Excellent organizational documentation and time management skills. Valid drivers license and reliable transportation for home visits. Willingness to work flexible hours to meet the needs of families including some evenings and weekends.
