Job Description
Qualifications:
1. Persons selfemployed or employed as Service Coordinators must be a/an:
- Licensed Master Social Worker (Licensed by the NYS Education Department);
- Licensed Clinical Social Worker (Licensed by the NYS Education Department);
- Individual with a Doctorate or Master of Social Work;
- Individual with a Doctorate or Master of Psychology;
- Individual with a Master of Gerontology;
- Physical Therapist (Licensed by the NYS Education Department);
- Registered Professional Nurse (Licensed by the NYS Education Department);
- Certified Teacher of Students with Disabilities (Certified by the NYS Education Department);
- Certified Rehabilitation Counselor (Certified by the Commission of Rehabilitation Counselor Certification);
- Licensed Speech Pathologist (Licensed by the NYS Education Department); OR
- Occupational Therapist (Licensed by the NYS Education Department).
An individual meeting any of the qualifications above must also have at a minimum one 1 year of experience providing case management/service coordination and information linkages and referrals regarding communitybased services for individuals with disabilities and/or seniors.
- Persons selfemployed or employed as Service Coordinators who have a Bachelors degree must have three 3 years of experience providing case management/service coordination information linkages and referrals regarding communitybased services for individuals with disabilities and/or seniors.
- Persons selfemployed or employed as Service Coordinators who have an Associates degree must have five 5 years experience providing case management/service coordination information linkages and referrals regarding communitybased services for individuals with disabilities and/or seniors.
Responsibilities:
Maintain active caseload
- Formally reviewing updating and submitting Service Plans to the RRDS for review (refer to the Service Plan section of this Manual for specific information on Service Plan responsibilities) and making sure all waiver service providers and the participant receive a copy of the all approved plans
- Conduct monthly facetoface meetings with assigned participants with at least one inhome visit no less than once a quarter
- Complete service plans as required by waiver manuals and addendums to meet the changing needs of the participant
- Assuring that the PRI/SCREEN is completed at least every twelve months; or when the participant experiences a significant improvement in his/her ability to function independently in the community
- Assist the participant in the development implementation and monitoring of all services in the service plan Maintaining records of waiver transportation as described in the Manual section on Transportation Services
- Maintain participant records in accordance with Waiver requirements
- Maintain knowledge of all approved waiver service providers in the region
- Organize and facilitate team meetings
Full time 40 a week hourly position travel within the capital district is required. Medical dental vision STD LTD Accident and Cancer protection life insurance are available. Free telehealth services. Paid time off and holidays!! Hourly pay between $21.00 and $23.00 per hour
