Job Description
The Case Management and Social Work team at UVM Medical Center is hiring for an Outpatient Social Work Case Manager II to join their team!
This position is onsite and will support our Outpatient Oncology Clinic.
JOB DESCRIPTION:
The Outpatient Social Work Case Manager (SWCM) is a comprehensive role within the interdisciplinary care team which delivers care to patients in a longitudinal rather than episodic fashion and which has as its focus: prevention; health promotion coordination and chronic disease management. The SWCM is responsible for assessment and intervention with any patient or family along the health care continuum where psychosocial and /or high risk factors are part of or interfere with the patients treatments or health outcomes. SWCM utilizes advocacy skills intervenes therapeutically and supportively provides resources and referral information and offers financial guidance and assistance as needed. The Social Work Case Manager will provide implementation sustainment and support of all youth young adult and adult screening as well as brief interventions and navigation to services through the SBIRT/SBIN Programs.
EDUCATION:
MSW from an accredited school of social work required. Licensure through the State of Vermont is preferred.
EXPERIENCE:
Greater than 1 year experience working in a human service organization health care setting or managed care system is required.
