Curant Health is searching for a Nurse Care Manager to join its team in Smyrna GA.
As the Nurse Care Manager we will look to you to perform care management duties to assess and assist patients and their support systems in managing chronic medical conditions more effectively to promote quality cost effective care.
Responsibilities

We have listed a few of your responsibilities below:
- Assess the patients current health status resource utilization past and present treatment plan and services and short and longterm goals.
- Assess and adjust the care plan and promote desired outcome.
- Provide patient and provider education.
- Enter and maintain assessments authorizations and pertinent clinical information into various medical management systems.
- Coordinate with patients Primary Care Physician (PCP) specialists medical providers and nonmedical staff as necessary to meet the needs of the patient (and clients).
- Provide proper followup and monitoring with patients and prescribers/healthcare providers.
Qualifications
- Bachelors degree in Nursing preferred
- 2 years of clinical nursing experience in a clinical acute care or community setting
- Demonstrated experience working with patients to coordinate and improve care.
- LPN or RN License required
