ABOUT THE PROGRAM: The Screening Brief Intervention & Referral to Treatment Prevention & Access to Treatment Hubs and Services (SBIRT PATHS)
The Screening Brief Intervention and Referral to Treatment Prevention and Access to Treatment Hubs and Services (SBIRT PATHS) initiative will serve youth ages 1221 with substance use and/or cooccurring disorders (SUD/COD) to ensure more youth access effective care in a timely way. The SBIRT PATHS initiative will establish a Connecticut (CT) communitybased model of cooccurring care for youth that addresses gaps in early substance use (SU) services prevents the progression of substance use disorders alongside other unmet social needs and facilitates connections between levels of care. This initiative will expand three evidencebased practices (EBPs) for youth including: Screening Brief Intervention and Referral to Treatment (SBIRT) Motivational Enhancement Therapy/Cognitive Behavioral Therapy (MET/CBT) and lowtouch Service Coordination using the Wraparound model.
Duties & Responsibilities
- Participate as member of multidisciplinary treatment team
- Track and coordinate client intake services and referrals as applicable
- Provide assistance to team including coordination of treatment planning and providing daily triage.
- Educate and link individuals to selfhelp groups natural support and other resources as needed.
- Assist with linking clients with community support systems and agencies and in obtaining benefits for which they may be qualified: insurance housing treatment representative payee legal services medical services education and employment.
- Maintain careful documentation of contacts and communication to maintain quality services. Complete all required documentation in compliance with agency policy.
- Work as a liaison between agency and other relevant providers and teams to coordinate care for referred clients.
- Assist with scheduling and providing transportation to and from appointments.
- Maintains documentation in compliance with agency and program standards.
- Participates in staff meetings supervision training and other meetings as assigned.
- Establish knowledge of cultures and communication styles associated with various groups particularly oppressed groups to provide culturally sensitive and competent care.
- Provides crisis intervention and support as clinically indicated.
- Additional duties as required.
QUALIFICATIONS:
Education: Bachelors degree in human servicerelated field required. Experience may be accepted in lieu of education for some positions.
Experience: A minimum of two years related experience preferred
Licensure/Certification/Registration: Complete any program specific training required. Complete required annual agency trainings. CPR/FirstAid Certification may be required within 6 months of hire.

COMPENSATION*:
$18.86/hour min and up commensurate upon experience.
*Actual rates are determined at the time of offer and are based off relevant experience as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or signon incentives. (Please note for parttime positions this range would be prorated based on hours worked)
