Your Opportunity

We are excited to announce an opportunity for a Fulltime Indigenous Health Worker/Practitioner to join our dedicated Maternal and Child Health team. In this rewarding role you will work closely with children and families providing essential care and support. The position offers a Monday to Friday work schedule ensuring a healthy worklife balance. We are looking for a compassionate experienced individual who is passionate about making a difference in the lives of Indigenous communities promoting health and wellbeing for both mothers and children. Your Contribution

This position is responsible for delivering direct care by promoting and coordinating clinical healthcare services for children and families within the Maternal and Child Health Service. It will ensure that children and families have access to culturally safe care and the necessary support systems.

Duties & Responsibilities

  • Assist in enabling the delivery of holistic and collaborative care of the individual/family to meet their needs and preferences during preconception pregnancy and perinatal period and for children and their families up to two years of age
  • Assist First Nations individuals/families to understand their options for support and planning across the stages of the First 1000 days
  • Assist First Nations individuals/families to identify their care goals and have these captured documented with collaboration from their TAIHS General Practitioner and other clinical care providers
  • Assist clients to access other supports and services internally and externally
  • Identify establish and maintain knowledge of and access to available community resources that support the philosophy of social and emotional wellbeing for First Nations people
  • Support the development and review of policies and work practices and support programs being delivered or implemented that address the social determinants of First Nations people
  • Support the development and implementation of culturally appropriate models of practice and support for First Nations people
  • Undertake follow up as required to ensure support is in place or assist to arrange alternative supports
  • Collect data and feedback that demonstrates outcomes and contributes to informing continuous improvement
  • Support the collation of data and statistics for routine and ad hoc reporting

Required Qualifications and Experience

  • Minimum qualification Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice or a qualification deemed relevant to the role
  • Welldeveloped understanding of the social detriments of health and social and emotional wellbeing affecting First Nations people
  • Ability to support and facilitate support groups and cultural groups
  • Demonstrated respect for Aboriginal and Torres Strait Islander people their history and culture
  • Proven ability to liaise with and support clients to access the services applicable to health and wellbeing
  • Proven ability to prepare and maintain reports and statistics associated with the role and responsibilities

Required Licenses and Checks

Ability to pass a National Police Check no Serious/Criminal/Court Records

Current Blue Card (Queenslands Working with Children Check) (or AHPRA registration if required)

Current C Class Drivers Licence (Qld)

Proof of qualifications and current registration (as appropriate) are to be provided prior to commencement of employment
