RN Case Manager
Job Location: Southern Maine
Compensation: $39$41 /HR
Description of the role:
The RN Case Manager will be responsible for coordinating managing and delivering quality patient care within the home health care setting. They will work closely with patients families and healthcare professionals to ensure appropriate care plans and interventions are implemented.
Responsibilities:
- Assessing patient needs and developing individualized care plans
- Coordinating and facilitating healthcare services
- Monitoring patient progress and making necessary adjustments to care plans
- Educating patients and families on proper selfcare techniques
- Collaborating with interdisciplinary healthcare teams
- Ensuring compliance with regulatory and agency standards
Requirements:
- Valid RN license in the state of Maine
- Prior experience in home health or case management
- Strong clinical assessment and care planning skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
