Position Overview:
The primary purpose of the Float Mental Health Worker is to provide direct support professional services and independent living skills trainings to the individuals of Residential Transitions Inc (RTI) in the Supportive Apartments Division. The Float Mental Health Worker is responsible for providing support to clients with independent living skills in multiple settings through use of a personcentered approach. The incumbent is collaborative attentive to detail has sound judgement and provides crisis deescalation response as needed. The Float Mental Health Worker works with a variety of internal contacts including program directors supervisors/ managers and mental health workers. All activities must support the mission of Residential Transitions Inc. to assist persons with disabilities in achieving the highest level of independence by providing an opportunity for homes in integrated community settings along with providing community services which assist individuals in meeting their desired goals. Role Responsibilities:
- Provides support and supervision of clients living in foster care and supportive apartments within guidelines of each clients goal plan Coordinated Service and Support Plan (CSSP) Individual Abuse Prevention Plan (IAPP) SelfManagement Assessment (SMA) PersonCentered Profile (PCP) and all other documents required by the 245D statute to provide client support. Incorporates the goal plan CSSP IAPP SMA and PCP into daily interactions with clients.
- Supports clients in mental health management through identifying coping skills practicing coping skills and alternative methods.
- Teaches and models community integration skills such as: self managing medications managing personal finances appropriate interpersonal interactions managing the public transportation system completing the necessary tasks to maintain a home accessing community resources and using and accessing natural supports appropriately.
- Models and responds with nonviolent interventions to crises including deescalation and motivational interviewing techniques to deescalate a persons behavior prior to posing an imminent risk of physical harm to self or others.
- Reports any medication errors or discrepancies to the site lead program supervisor/manager and area director follows up with physician and follows up with medication error reporting guidelines.
- Provides support to program sites with administrative functions that include charting cleaning collaborating with other providers participating in team meetings problem solving and communicating updates with necessary external stakeholders.
- Monitors clients for symptoms of decompensation crisis and intervenes when necessary. Provides guidance and assistance to mental health workers across supportive apartment division when symptoms are identified and responds in emergency medical situations. Completes charting and notifies the appropriate parties following medical matters.
- Interacts informally with clients to facilitate socialization and appropriate behavior while also maintaining professional boundaries always.
- Teaches healthy living habits and develops the accompanying skills; participates in scheduled and Lead Float Mental Health Worker Page 2 of 3 spontaneous recreational events.
- Maintains awareness and knowledge of RTI program changes and development
- Notifies applicants and other involved parties regarding admissions criteria recommendations and decisions
- Ensures documentation client files statistics and database entries are accurate and updated regularly.
- Assists program managers with ensuring compliance with 245D licensing requirements clienttostaff ratios and incident reporting guidelines at each site.
- Assists program managers with medication observation with staff medication protocol review training new mental health workers and other site reviews or training as needed. Assists program managers with training employees on program policies and rules. Clarifies policies and procedures for the clients. Charts logs and reports orally as required by program policy.
Required Qualification and Experience: - Must possess a minimum of one 1 year of experience working with persons diagnosed with Severe and Persistent Mental Illness (SPMI). Supervisory experience is preferred. Bachelors degree in human services Psychology Social Work Sociology or a related field is preferred. An equivalent combination of education and experience may be considered.
- Must pass the required background check and remain eligible to provide services as defined by the Minnesota Department of Human Services.
- Must pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
- Must have access to a reliable vehicle and maintain a valid drivers license and insurance
- Other as needed as company and role expand or evolves.
