Job Summary:
The City of Greeley is seeking a Foundations Team Supervisor who is passionate about Homeless Solutions and Community Resources.
The individual in this role will be responsible for being the direct supervisor of the Homeless Solutions Foundations Team at the City and providing oversite of the utilization of rental assistance vouchers and other subsidies. Ensures best practices such as Housing First Harm Reduction and Trauma Informed Care etc. are utilized and incorporated at all levels of the teams position will have a small case load and work with clients to increase income ensure health insurance coverage and help those who are able to work secure employment. The Foundations Team Supervisor will provide support advocacy safety planning and counseling to individuals experiencing homelessness. In addition the Foundations Team Supervisor will provide frontline supervision to the entire case management team by providing scheduling for the team identifying solutions for clients and problemsolving obstacles when they arrive. The team manager will ensure programming guidelines are followed proper documentation is being kept and assist with training in the areas of HMIS. This Position will report to the Program Officer (title) but will work closely with the Clinical Manager for joint supervision of the case management teams.
Experience Knowledge Skills:
Minimum Requirements
- Bachelors degree in behavioral health field (Sociology Psychology Social Work Criminal Justice etc) or unrelated field with one year of documented work in behavioral/social services.
- Demonstrated experience in managing Housing First Intensive Case management.
- Extensive experience working with and servicing people with very complex needs including but not limited to chronic homelessness serve trauma substance use disorders mental illness.
- One 1 years of supervisory experience.
OR
- Any combination of related education experience certifications and licenses that will result in a candidate successfully performing the essential functions of the job.
Preferred
- Bilingual in Spanish and English.
- Housing first services experience.
- Masters degree in support services or related field of study.
- Exceptional skills in Microsoft Word Excel Access PowerPoint or other equivalent software packages.
- Six 6 months of experience working within a municipality.

- Knowledge of the business and organizational structure of Colorado municipalities

Essential Functions:
- Ensures tenant based rental assistance vouchers available to clients and service programs are utilized to the maximum extent to expand the number of clients housed and maximize subsidy revenue to properties.
- Completes coordinated intakes assessments and screenings to determine housing eligibility based on HUD and
Colorado State Department of Housing (DOH) guidelines
- Very familiar with HMIS and understands the need to have strong data to tell our stories. Assist case managers with proper documentation in HMIS.
- Assist Case Managers to completes strengthsbased housing plans that identify barriers to housing and set a path to housing stability; reviewed and updated monthly
- Supervise managers to provide case management services focused on housing increasing income establishing and/or expanding family and communitybased support and assisting in increasing access to mainstream services
- Ensure best practices such as Housing First Harm Reduction and Trauma Informed Care etc. are utilized and incorporated at all levels of the teams functioning.
- Oversees the scheduling for the foundations team and ensures coverage at all times. Will serve as back up when short staffed and be on call as needed
- Leads Daily huddles for the Foundations Team
- Ensures coordination with the HMIS system in receiving new referrals and is responsible for determining program eligibility and funding source and assigning participants to vouchers and participants to Case Managers
- Assists in program development and quality assurance activities as directed by the Deputy Director and Director
- Supervisors Case managers with locating available housing units the leasing process landlord mediation and tenant/landlord rights and responsibilities
- Attends Homeless Solutions training and workshops related to housing
- Attend monthly case management meetings meetings with supervisors and staff meetings as scheduled
- Cultivate new and existing partnerships in Greeley and the surrounding area that will increase client skills and establish goals (life management skills educational and/or career goals financial management skills health and wellness goals and decisionmaking skills)
- Responsible for reading all logs/emails in a timely manner to ensure you are up to date with current information pertaining to the residents
- Performs other duties as assigned
