Health Related social needs (HRSN) Navigator (HYBRID)

JOB SUMMARY

The Health Related Social Needs (HRSN) Navigator provides screening and case management Services for eligible (Medicaid Managed Care) candidates through the Social Care Network program. The HRSN Navigator will be responsible for linking people who qualify for Enhanced HRSN services to approved support to help improve health related outcomes. The HRSN Navigator will also be responsible for developing implementing and updating HRSN Care Plans for people enrolled in Navigation Services.

ESSENTIAL FUNCTIONS

  • Provides Screening using multiple means (Phone Teams In person) for preselected Medicaid Recipients identified by Person Centered Services and other agencies through the Social Care Network (SCN) in a positive and professional manner.
  • Manages confidential information in an Electronic Health Record (HER).
  • Completes referrals and links eligible people to Enhanced HRSN Services and provides HRSN Case management to address their identified need(s).
  • Develops and delivers outreach materials/training opportunities for internal and external Care Coordinator partners.
  • Utilizes specific SCN platforms to conduct activities retrieve reports. And complete the billing requirements for the services rendered.
  • Manages implements and maintains HRSN Social Care plan.
  • Responsible for creating reports related to screening HRSN Navigation Services and to other items related to SCN.
  • Collaborates with SCN partners and other providers on services available and process and procedures.
  • Collaborate and share information with care coordinators and intake team members as appropriate.
  • Link people to services that meet their identified needs.
  • Ability to effectively manage a caseload.
  • Completes documentation in a timely manner.
  • Ability to travel and meet with individuals in their homes physician/provider offices and other public places to conduct screening and to provide services.
  • Reliable transportation.
  • Flexible schedule (may include after hours and weekends).
  • Monitor satisfaction with support and services.
  • Ensure case files are following regulations and policies.

OTHER DUTIES

  • Basic computer skills include PowerPoint Outlook Word and Excel.

  • Knowledge of confidentiality regulations.

EDUCATION & EXPERIENCE

  • High school diploma with experience in a customer support role required.
  • Associate degree or bachelors Degree preferred.
  • One year experience working with people with Developmental disabilities preferred.
  • Bilingual in Spanish preferred.
  • Valid Drivers License required

The pay rate for HRSN is $23.50 per hour and for Spanish Bilingual candidates the pay rate is $25.00 per hour.
