The Segue Residential Transition Program assists with helping individuals transition out of staterun psychiatric hospitals. Segue clients are clinically stable and ready to leave the hospital. The services provided assist clients with learning how to manage their illnesses and develop independent living skills.
What to expect.
You will oversee program development coordinate care and supervise staff with specific focus on the daytoday operations of the inreach and service coordinator teams.
Supervisory responsibilities include:
- Overseeing all inreach processes ensuring services are authorized and compliant with guidelines.
- Supervising staff and ensuring effective onboarding training and development.
- Assisting in the development and management of the program budget policies procedures and quality improvement systems.
- Maintaining compliance with licensing accrediting and funding standards and implementing corrective action plans as needed.
Clientfacing responsibilities include:
- Conducting comprehensive clinical assessments developing treatment plans and implementing effective interventions for individuals with serious mental illness including those with cooccurring disorders.
- Supporting clients involved in the forensic system ensuring compliance with conditional releases probation requirements and collaboration with mental health courts.
- Developing and implementing strategies to support individuals with mental illness in transitioning successfully from longterm hospital stays to community living.

The pay range for this position is $55000 minimum to $65920 maximum. Pay for this position is determined on a number of factors including but not limited to years and level of related experience.
What we need from you.
- One of the following combinations of education and experience:
- A high school diploma or GED and 5 years of experience.
- An associates degree and 4 years of experience.
- A bachelors degree and 2 years of experience.
- A drivers license with 3points or less and access to an insured vehicle during work hours.
- Candidates must possess or be able to obtain the CPRP certification by July 1st 2025
