A Day in the Life of a Palliative Care Coordinator

The Palliative Care Coordinator is a pivotal nursing role focused on the development and management of palliative care services ensuring the program aligns with regulatory standards and operational goals. This role involves direct patient interaction from initial diagnosis through the entire care continuum involving assessing needs managing symptoms providing essential information and supporting patients and families. Challenges include maintaining highquality care while coordinating with multidisciplinary teams including physicians medical staff and external partners to optimize patient outcomes. Collaboration is key as the coordinator acts as a liaison among various stakeholders and advocates for patients in interactions with insurance entities and regulatory bodies. The structure includes developing policies overseeing staff training and continuously improving program quality based on the latest palliative care trends and regulations.

About the Hospital Care Management Department:

In Hospital Care Management at CRMC we ensure that patients receive the right care at the right time in the right settingeven after they leave our hospital. From coordinating discharge plans and arranging followup care to addressing social determinants of health our team ensures a smooth transition from hospital to home rehab or other care facilities. We are patient advocates problem solvers and resource navigators ensuring that no patient falls through the cracks. Qualifications

Education and Experience

Required

  • Two years of experience in Palliative Care
  • Current Minnesota RN license
  • American Heart Association BLS within 30 days of hire
  • 5 years experience in acute care
  • Palliative Care Certification within 3 years of hire
  • Valid Minnesota drivers license
  • Bachelor of science/art in nursing

Preferred

  • Masters degree in Health Care related field (Nursing Social Work Health Care Administration etc.

Compensation and Benefits

The pay range listed above is the estimated starting pay range for this role. Actual pay rate offered depends on many factors including each individual employees experience level preferred education/certifications along with market conditions.
