Job Details
The (PIH) Care Coordinator will function as a member of the Pediatric Integrated Health (PIH) Care Team providing direct and indirect care coordination services to client/family. In addition this position supports transitional living processes for client. This role will provide linkage followup and support of behavioral physical and supportive care for PIH client/family. While also having a good understanding around community based resources eligibility and points of access.
Qualifications
Qualified Candidates
- Bachelors degree in social work or related human services field.
- Minimum of oneyear relevant postdegree experience in the human services field.
- Any equivalent combination of experience and training which provides the knowledge and abilities necessary to perform the work.
Required Special Qualifications:
- Must possess a valid Drivers License and meet the driving requirements of the Agency.
