Case Manager PHAP
Peer Assistance Services Inc. is a communitybased 501(c)3 nonforprofit Colorado corporation. PAS implements two statewide programs Screening Brief Intervention Referral to treatment (SBIRT) and the Peer Health Assistance Programs (PHAP)
Screening Brief Intervention and Referral to Treatment or SBIRT is an evidencebased early intervention practice to identify reduce and prevent use of alcohol and other substances. SBIRT calls for communitybased screening for health risk behaviors including substance use. SBIRT in Colorado is a statewide program that provides training technical assistance and implementation support to health and mental healthcare providers healthcare organizations and stakeholders. SBIRT in Colorado manages a statewide campaign to increase public awareness of the risks related to alcohol and other drug use and to reduce the stigma of seeking treatment.
Peer Health Assistance Programs contract with the Colorado Department of Regulatory Agencies Division of Professions and Occupations to provide the Peer Health Assistance Programs which are statutorily created and funded by license fees. These programs provide assessment referral longterm case management and monitoring for health and mental health professionals with physical emotional psychiatric psychological and/or substance use conditions affecting their practice.
These are the professions who have statutorily created programs for Nurses RNs and LPNs Direct Entry Midwives Dentists and Dental Hygienists Pharmacists and Pharmacy Interns Veterinary Professionals Marriage and Family Therapists Professional Counselors Psychologists Social Workers Addiction Counselors and Unlicensed Psychotherapists.
The Peer Health Assistance Program is available on a fee for service basis to other licensed healthcare providers in Colorado who do not have a statutorily created program. These professions include physical therapists surgical technicians/assistants chiropractors optometrists nursing home administrators podiatrists athletic trainers massage therapists occupational therapists respiratory therapists and speech and language pathologists.

About the Case Manager Position
This is a handson fulltime position with daytoday responsibilities that will include ensuring program participants are able to practice their profession with reasonable skill and safety while complying with regulatory requirements. You will also collaborate with licensed evaluators to develop a monitoring contract outlining treatment and community resources to optimize program participants physical and mental health and wellbeing. You will need to maintain regular communication with program participants treatment providers and workplace supervisors to measure compliance with the monitoring contract. You may also have to adjust monitoring contract requirements based on compliance measurements while keeping accurate records of contract monitoring throughout the program participants involvement in the Peer Health Assistance Program. Your goal will be to help program participants achieve optimal health and wellbeing while also meeting regulatory requirements.
Compensation:$55000 to $60000
