Developmental Pathways (DP) has an opportunity for a Case Manager (CM) Enrollment in our Systems Navigation (SN) Department.The Enrollment CM will monitor and confirm that functional and financial eligibility requirements for individuals are met. This role will complete assessments support plan development and coordination process with Interdisciplinary Team (IDT) and comprehensive care team. The Enrollment CM connects identified needs with services and supports available to ensure a personcentered approach to support plan development and will ensure eligibility for desired services is secured and maintained. Also will assist with Intake duties as needed.
This role offers a 4day 40hour workweek Tuesday Friday and includes the flexibility to work remotely as well as the opportunity to collaborate inperson at our headquarters office in Aurora CO and/or at offsite locations.

Five 5 years of experience in the field of LongTerm Services and Supports; or
(LTSS) which includes Developmental Disabilities; or
Some combination of education and relevant experience appropriate to the requirements of the position
Valid Colorado motor vehicle license and provide proof of motor vehicle insurance
Meet our driving requirements no suspensions and no more than two 2 tickets in the past three 3 years
Upon request qualifying need and approval driving requirements may be waived in lieu of reliable transportation.
Preferred
One 1 year of experience in developmental disabilities/longterm services and supports
Additional Responsibilities
Serve as subject matter expert for applicable programs and services; serves as resource/point of contact for complex cases
Assess the overall needs of an individual and assist with the exploration of programs longterm services and supports and other community resources to meet assessed needs
Assist individuals and families in maintaining eligibility through the completion of required assessments monitoring and support planning
Provide care coordination services and interventions by referring educating negotiating and mediating with the individual and external providers/agencies
Complete accurate case documentation in appropriate tracking and databases; prioritize workload and meet all required timelines
Complete required documentation related to monitoring activities; evaluate need and effectiveness of services requested and delivered
Schedule facilitate document and complete followup for all required and adhoc meetings; disseminate updates to the necessary parties
Ensure the accuracy and timely entry and distribution of required documentation to all necessary parties including but not limited to providers individuals and family members
Maintain knowledge of state/ federal regulations policies and procedures regarding longterm services and supports Medicaid waiver and statefunded programs and other community resources
Maintain a professional and ethical manner in all interactions and meet performance quality customer service and coordination standards as assigned by department leadership; maintain appropriate boundaries
