Job Summary:
The Care Coordinator will determine eligibility of persons applying to receive assistance from government programs and clinic resources and provide support for services needed.
Key Responsibilities:
- Meet with new and established Medicaid patients to answer questions and provide information about the clinics and services.
- Schedule appointments for additional services for patients when meeting with them.
- Provide followup to patients to address barriers to care and answer questions.
- Assist patients with connecting to other resources.
- Assist patients who have difficulties completing paperwork or understanding information.
- Serve as a continuity figure to patients.
- Assist in providing outreach and Medicaid enrollment within the community.
- Assists patients with applications to Medicaid which also induces temporary Medicaid presumptive Medicaid disability Medicaid pregnancy Medicaid and verifies insurance.
- Medical and Prescription Drug Coverage: Three comprehensive plan options (Buyup Base and High Deductible) through UnitedHealthcares Choice Plus network covering various deductibles and outofpocket limits. Includes access to telemedicine services via Teladoc.
- Health Savings Account (HSA): Available for employees in the High Deductible Plan with employer contributions and tax advantages.
- Flexible Spending Account (FSA): Options for both healthcare and dependent care FSAs allowing pretax contributions for qualified expenses.
- Dental and Vision Coverage: Dental insurance through Cignas DPPO network and vision coverage through EyeMeds Insight network.

Preferred Qualifications:
