PACE (Program of AllInclusive Care for the Elderly) Nurse Practitioner

Our ideal Nurse Practitioner candidate will provide and manage comprehensive primary care in the participants place of residence and in the clinic as appropriate which may include but is not limited to routine checkup wound care procedures and lab/sample collection (consistent with level of training and credentials).


PACE NP must work independently and must be responsible selfconfident and have excellent judgment and decisionmaking skills focusing on elderly patients and programs. Nurse Practitioner must demonstrate respect and regard for the dignity of all patients families visitors and fellow employees to ensure a professional responsible and courteous environment.


Duties:

  • Take medical histories and perform patient examinations.
  • Order and interpret laboratory tests and xrays.
  • Make diagnoses.
  • Provide treatment.
  • Manage chronic health conditions such as diabetes.
  • Perform preventive procedures such as immunizations.
  • Prescribe medications and other therapies.
  • Monitor and record patient progress.
  • Educate patients about their health and instruct them in selfcare techniques.
  • Promote wellness practices.
  • Respond to medical emergencies.
  • Performs work of equal skill and responsibility as directed. May also perform work of a higher level in preparation for increased responsibility and may be requested to perform work of lesser responsibility when workload of the department so requires.
  • Refer patients to other physicians and other health care providers as needed.

Requirements
  • Masters Degree in nursing with an emphasis in nurse practitioner
  • At least two years as a nurse practitioner preferred
  • Elder care experience a plus

Required Licenses/Certifications:

  • Missouri State Advance Practice Nurse License
  • Current BLS Certification required.
