Responsible for managing all aspects of the patient intake process including coordinating patient admissions to Home Health and Hospice and communicating to the staff all pertinent information regarding new and returning patients from the hospital and clinic physicians.
Acts as the resource person for patients families caregivers physicians and referral sources; establishing and maintaining positive relationships with customers and referral sources responding to customer requests and concerns and managing the insurance verification and authorization processes.
MINIMUM QUALIFICATIONS AND REQUIREMENTS

Active LPN in good standing

Experience Requirements
Experience in home health and hospice care operations preferred.
Demonstrates good communication negotiation and public relations skills.
Demonstrates autonomy organization assertiveness flexibility and cooperation in performing job responsibilities.
Special Skills or Training Requirements
Demonstrated ability to assist professional and administrative personnel.
Ability to market aggressively and deal tactfully with customers and the community.
Knowledge of corporate business management governmental regulations Joint Commission standards and private payer practices.
