UnityPoint Health is embarking on an exciting and innovative care delivery model. Our new Collaborative Care Model (CCM) enables all members of the multidisciplinary care team to practice at the top of their scope leveraging your unique skills training and strengths.

As an LPN in this care model you will provide care for a specified group of patients utilizing all the skills you have gained in obtaining your license. With the support of an RN and other team members of the care group you will perform tasks or skills within your scope of practice including but not limited to:

  • Provide safe and reliable patient centered care with support and supervision of an RN.
  • Collaborate with multidisciplinary team members.
  • Manage care with ongoing assessments and vitals.
  • Administer medications as ordered by the provider.
  • Provide evidencedbased patient education starting at admission.
  • Document daily progress toward care plan goals.
  • Complete bedside procedures (e.g. foley NG dressing changes drains etc..
  • Provide discharge education and review instructions with patient and/or caregivers.
  • Document in the electronic medical record

Recognized as a valuable member of the Acute Care team the LPN practices under the supervision of the Registered Nurse and assists patients while providing a therapeutic environment for patients and their families. Responsibilities

Clinical Excellence & Patient Care

Possesses the knowledge and clinical skills necessary to provide care for people with physical disability and chronic illness

Performs handson nursing care by utilizing the nursing process to achieve quality outcomes for patients

Reinforces the care provided by other team members

Provides direction and supervision of unlicensed assistive personnel (i.e. Patient Care Technician) demonstrates clinical judgment appropriate for Iowas LPN scope of practice uses problem solving techniques and timemanagement principles and delegates appropriately

Demonstrates effective oral and written communication skills to develop a rapport with patients their families and health team members and to ensure the fulfilment of requirements for legal documentation and reimbursement

Contributes to the assessment of the physical psychological sociocultural and spiritual dimensions of patients and their families as well as their educational and discharge needs

Contributes to the evaluation of the nursing care and to the modification of the plan as needed to achieve measurable goals and objectives

Intervenes with team members and other healthcare professionals to ensure that the optimal opportunity for recovery is made available to the patient.

Collaborates with the staff nurse and other team members to achieve costeffective care by utilizing appropriate clinical measures to meet emergent physical psychosocial and spiritual situations

Communicates effectively and interacts in a manner that facilitates the group process and team building.

Offers input into team decision making within their scope of practice.

Advocates and supports the rights of patients and families with care decisions.

Intervenes on behalf of patients along with the staff nurse to ensure that medical and nonmedical professionals work to maximize patients success at time of discharge and beyond.

Practice reflects knowledge of federal/state regulatory and accrediting agencies i.e. Commission on the Accreditation of Rehabilitation Facilities (CARF) and the Iowa Department of Inspection and Appeals (DIA).

Chooses an appropriate and meaningful communication style when interacting with others; gives and accepts constructive feedback to/from peers

Ensures patient safety in the hospital environment by following policy/procedures and adhering to National Patient Safety Goals

Consults appropriate resources within the hospital to provide specialized education

Utilizes health patient education system and other resources for patients and family members

Completes discharge education with patients and/or caregiver.

Utilizes teachback method of education.

Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

This position may be assigned a specific mission to address in an emergency situation to assist in disaster response and recovery as part of the UPHDM Hospital Emergency Incident Command System. Situations of a critical nature may require an individual to perform multiple tasks until additional support can be obtained.

Education:

Successful completion of a licensed practical nurse program from an accredited college.

Experience:

Experience or clinical time in PostAcute Care or an Acute Care environment.

License(s)/Certification(s):

Current licensure in good standing to practice as a Licensed Practical Nurse in Iowa.

Must possess and maintain current Basic Life Support (BLS) certification or obtain certification within three 3 months from date of hire.

Valid drivers license when driving any vehicle for workrelated reasons.

Knowledge/Skills/Abilities:

Writes reads comprehends and speaks fluent English.

Multicultural sensitivity.

Microsoft Office basic computer skills.

Customer/patient focused.

Critical thinking skills using independent judgment in making decisions.

Other:

Use of usual and customary equipment used to perform essential functions of the position.
