Protective Services Care Manager Key Performance Objectives Include:
- Obtains information and investigates complaints/reports of alleged abuse neglect or exploitation of theelderly.
- Participates in taking required corrective action in compliance with the Pennsylvania Older Adult Protective Services Act.
- Performs homebased assessments/reassessments investigations and care management for assigned participants to identify participant needs and facilitate effective utilization of resources.
- Collaborates with participant and/or caregiver in the development of a care plan which promotes the participants highest level of functioning in the least restrictive community environment.
- Recommends cases for court intervention including guardianships or mental health commitments. Testifies in court concerning various aspects of assigned cases.
- Provides ongoing extensive care management and supportive services to participants through home visits and contact plans as required by PA Department of Aging (PDA) to maintain medical financial and emotional stability.
- Provides or arranges for needed supportive services and coordination with other agencies to fulfill service plan objectives for highrisk participants.
- Demonstrates knowledge of supportive and specialized services provided through Challenges Options in Aging programs Pennsylvania Department of Aging and local community agencies/resources through appropriate intake screening and referral of participant inquiries.
- Maintains participant records in Social Assistance Management System (SAMS) in a timely manner to ensure program and fiscal accountability and compliance with Pennsylvania Department of Aging requirements.
- Provides community outreach by establishing consistent visit schedules at local hospitals housing authority facilities rural township offices and other location determined by need to provide older adults/families with information needed to make fully informed decisions about remaining in their homes. Determines contact for facility or location. Establish relationship with the contact in order to provide services.
- Delivers educational presentations/training about protective services and the support available from Challenges Options in Aging to outside groups organizations and businesses.
- Maintains confidentiality in communication with participants staff and others to preserve client and staff rights.
Minimum Qualifications: - Two years of experience in public or private social work; and a Bachelors Degree is required; or any equivalent combination of experience and training including successful completion of 12 semester hours of college level courses in sociology social welfare psychology gerontology or other related social sciences.
- Excellent oral and written communication skills including interviewing and assessment techniques are required.
- Knowledge of the aging process and geriatric issues is highly desirable.
- The demonstrated history of successfully interacting constructively with community agencies and resources is required.
- The ability to make home visits is required.
- The ability to work a rotating oncall shift once a month is required.
- A valid drivers license is required.
- Act 33/34 clearance required.
