Utilizes teachers to coordinate and deliver education for patients at risk academically or identified as having a disability health and/or mental health condition that impacts their learning.
Essential Functions:
- Identifies potential candidates for school services from hospital census team referrals and other sources; communicates information about school program to the parents and gathers necessary information and consent for services.
- Assesses individual educational needs selects educational goals and provides educational activities for schoolaged and adolescent students in collaboration with family team members and home school districts.
- Communicates with NCH school liaison and school districts/teachers to provide instruction based on districts curriculum to understand and support the childs individual education plans (if applicable) and to facilitate the childs enrollment or reenrollment in an appropriate homeschool program.
- Provides parent information about the hospital school program and community resources learning needs of child and educational options.
- Completes reports/records/forms as required by the hospital in a timely and thorough manner. Attends and contributes to interdisciplinary team meetings as needed.
- Educates the hospital staff and community on the role and importance of school services within and outside of the hospital.
Education Requirement:
- Bachelors Degree in Education Health Services Human Development Social Work or related healthcare field required.
- Masters Degree preferred.
Licensure Requirement:
- Valid Ohio State Board of Ohio Department of Education Professional or Resident Educator License required.
- Intervention Specialist preferred.

Experience:
Experience in educational or clinical setting preferred.
