Job Summary
TheCase Managerprovides case management services in the community to patientsexperiencingserious mental illnessand/or those who are considered high risk due to their acuity level.The Case Manager assists in planningconnecting advocacy coordination and monitoring; to assist patients in gaining access to health services financial assistancehousingemployment education social services and natural supports.They assesneeds develop and monitor individualized treatment plans and document services provided in an accurate and timely manner.The Case Manager embraces the recovery philosophy and promotes a safe recovery environment for clients. The Case Manager will be responsible for implementing advanced techniques and interventions to serve the highrisk individuals in their associated program.
Skills/Requirements
Highschool diploma or GED and either:
o 18 credit hours of posthigh school education in a field related to behavioral health and two years of fulltime behavioral health work experience; or
o four years of fulltime behavioral health work experience.
Has an associates degree and at least two years of fulltime behavioral health work experience; or
Has a bachelors degree in a field other than behavioral health and at least one year of fulltime behavioral health work experience; or
Has a masters degree or bachelors degree in a field related to behavioral health.
Minimum of ninety 90 days experience in a Navigator or Peer Support Specialist role required.
Minimum of six months of recovery from substance use and/or mental health disorders required.
Current AZ Drivers License (valid and in good standing).
Clear 39month Motor Vehicle Record.
Arizona Fingerprint Clearance card (program specific)
