Job Description

We are working closely alongside a Local Authority in Enfield to assist with the appointment of a Reablement Officer on a 3month contract highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: 15.50 19.06 per Hour

Hours Per Week: 28.00

Summary:

Enfield Council is seeking a dedicated and experienced Reablement Officer to provide highquality personcentred support to service users helping them work towards achieving independence and health outcomes. The successful candidate will work collaboratively within the Reablement/Assessment and Prevention Service to promote service users autonomy through shortterm goaloriented interventions.

Responsibilities:

  • Work closely with service users to assess plan implement and review care plans in collaboration with the therapy team.
  • Deliver practical support in service users homes to help them regain skills for daily living.
  • Maintain accurate and uptodate electronic records in line with data protection and departmental policy.
  • Act as a key/case worker as required and support advocacy access for service users.
  • Foster relationships with internal teams external agencies families and carers to ensure cohesive care planning and implementation.
  • Support service users inclusion and participation through group work and other interventions.
  • Participate in induction training and team development activities.

Qualification:

  • NVQ/QCF or equivalent level 3 in Adult Social Care

Essentials:

  • Minimum of 2 years experience supporting vulnerable adults in home/community settings.
  • Strong IT competency for electronic record keeping.
  • Ability to assess service users and track progress toward reablement goals.
  • Excellent communication skills particularly in spoken English.
  • Full clean driving licence and access to a vehicle for boroughwide travel.

  • You should be available to work immediately or at a short notice.
  • You should have right to work in U.K
  • This role requires an enhanced DBS
