Job Description
The Care Coordinator is the main point of contact for our clients their family members and our Caregivers regarding care needs and changes. As the Care Coordinator you will be responsible for supervising the care of our clients by assessing and monitoring the quality of services provided to them in addition to changes in condition such as illness injuries hospitalizations etc. and managing updates in their Care Plan. This will be accomplished through introductory visits and consistent onsite visits which are an essential role in the Care Coordinator position. The Care Coordinator is also responsible for ensuring all Caregivers are prepared to care for our clients based on their skill level and the clients care needs.
The Care Coordinator is also responsible for identifying interacting with and maintaining key referral sources alongside our Business Development team. They will create a bridge between key referral sources including case managers discharge planners administrators and others who work within skilled nursing senior living communities hospitals physician offices home health and hospice. Through their daytoday duties the Care Coordinator is expected to meet and exceed their goals to boost the support of our agency.
Duties include:
Conduct introductions and supervisory visits including 30 day 60 day 90 day and quarterly visits of clients and caregivers
Assist with prospect calls and assessments as needed
Maintain professional relationships with current referral sources
Identify and pass off information from a sales standpoint for our Business Development team to handle
Assist the Business Development team in scheduling and executing community marketing events including health fairs senior expos and other events that promote our services to the general public;Attend 1 networking event or 1 community event a month
Document in a timely manner all notations in our homecare software program
Ensuring Caregiver issues are raised to the appropriate staff in a timely manner
Perform caregiver and client introductions upon first visit or on next business day
Assist with new hire orientation training quarterly trainings and ongoing trainings on a consistent basis
Manage and assemble client resource folders and engagement bags to be delivered at each introductory visit
Responsible for sharing feedback regarding best matches and schedule times of clients to Scheduling Coordinators
Creates client care plans through completed assessments
Follows up on incident reports hospitalizations and changes in condition as needed within 24 business hours of occurrence
Provide outstanding customer service at all times to all clients & family members referral sources VOSH staff and caregivers
Participate in all quarterly caregiver training meetings including organizing speakers through community partners
Occasionally fill in on shifts when needed
Roughly 50 office work and 50 field work
Education Experience and Certifications
Job Requirements:
Access to a reliable vehicle at all times and ability to drive throughout your day
Proven ability to supervise train and mentor others
Minimum 2 year experience providing care to seniors or persons with disabilities
Proficiency with Microsoft Office Outlook and web applications
Excellent organizational customer service telephone and interpersonal skills
Ability to connect and interact clearly with seniors
Ability to work safely while performing job duties
Must have superior oral and written communication skills
Ability to work under pressure and meet deadlines
Office hours MF from 8am to 5pm with flexibility to manage duties before and after hours as needed
Rotation of OnCall weeknights and weekends as needed

Salary: $46000$52000 base salary based on experience with quarterly bonus and full benefit options.
