*Work is performed in an interior office environment within a Low Barrier Shelter.

JOB SUMMARY:The Bilingual Case Manager provides direct case management services to clients residing in the Low Barrier Shelter. The Bilingual Case Manager provides onsite case management services including creating individualized goals to assist clients to swifly exit the shelter document client progress in the appropriate database within 48 hours identify and address client barriers to housing and/or employment connect clients to benefits eligible housing resources and community referrals; work collaboratively with program team to increase positive shelter exits for shelter guests.

ESSENTIAL DUTIES and RESPONSIBILITIES:

  • Interview clients for direct service needs and develop a case plan with the client including written shortterm and longterm goals and Make referrals as needed.
  • Conduct ongoing assessment of client needs and reevaluate goals with clients at regular.
  • Facilitate daily crisis management acting as a liaison between client and clients environment.
  • Collaborate with other disciplines including substance abuse legal medical educational vocational and mental health professionals and other service providers.
  • Make appropriate referrals for emotional substance abuse legal medical dental educational recreational vocational employment and housing.
  • Create and maintain case records including but not limited to legal documentation assessments case notes case plans service updates court reports medical reports referrals discharge summaries visitation records and educational information in designated databases such as the Homeless Maintenance Information System (HMIS) QuickBase and STEP within contract requirement timelines.
  • Enhance the client environment by creating a hospitable and customeroriented service.
  • Complete reports and maintain statistical data meet programmatic documentation and other performance standards on an ongoing basis.
  • Perform all other jobrelated duties as assigned.

EDUCATION and EXPERIENCE:

  • Bachelors degree in human services or a related field
    • If with a Social Work degree Social Work licensure in the appropriate jurisdiction is required.
    • If with a Professional Counseling degree Professional Counseling licensure in the appropriate jurisdiction is required.
  • One 1 year of experience working with atrisk clients in direct services required (internship experience applicable).
  • Experience working with clients who have a history of homelessness addiction and/or mental health issues. In some cases 2 years of work experience in social services can be substituted with additional related certifications or a masters degree in human services or a related field.

SKILLS and COMPETENCIES:

  • Excellent communication both verbal and written in English and Spanish.
  • Ability to prepare clear concise and accurate reports correspondence and other jobrelated.
  • Ability to analyze situations accurately utilizing a variety of analytical techniques to make wellinformed.
