Job Description

We are eager and ready to hire another Clinical Pharmacist to join our growing practice. The new clinician will join a growing group of patient centered clinical pharmacists in Southwest Washington. Our Pharmacy Services Department currently consists of a small group of pharmacists and pharmacy technicians. Our pharmacy team provides direct patient care for patients in SW Washington and Portland Oregon. Our pharmacists are involved with our Pharmacy and Therapeutics Committee Epic optimization of medication orders and tools management of multiple population health initiatives and providing drug information and cost containment strategies for patients and the organization. The Clinical Pharmacist Specialist will:

  • Be responsible for performing all necessary functions to ensure optimal use of medications to achieve specific outcomes that improve a patients quality of life.

  • Serve as a clinical lead in population health and be a strong contributor towards the Vancouver Clinic goal of valuebased care by providing focused multidisciplinary support for patients with chronic or highrisk medical conditions to optimize and streamline medication regimens to current standards of care.

  • Provide direct patient care and be responsible for managing the pharmacotherapy of complex patients with extensive Collaborative Drug Therapy Agreements with physicians at the Vancouver Clinic.

  • 1.0 FTE position located in clinic offices in Vancouver and Portland with no weekend evening or holiday hours.

  • Be capable of seeing up to 15 patients a day through inperson and telehealth appointments.

  • Have an active Washington license.

The ideal candidate is an outgoing hardworking patientoriented team player with an excitement to practice evidencebased medicine in a collegial cohesive group of other clinicians. They will be excited to work sidebyside with physicians and advanced practice clinicians. Preference will be given to candidates with residency training 3 years of experience and/or comfort with comanaging care with other clinicians. Newer ambulatory trained pharmacists that are quick learners and have excellent interpersonal skills are also encouraged to apply. Our clinical pharmacists are enthusiastic teachers and are eager to provide mentoring and onthejob training for the right longterm candidate.

We are rooted in Southwest Washington located in the beautiful Pacific Northwest. The area offers a variety of housing options from suburban neighborhoods downtown high rises or rural locations with acreage.
