Salary

The salary range for this position is CAD $42.56/Hr. CAD $52.95/Hr.

Summary

Reporting to the Clinical Operations Manager and receiving professional practice leadership and support from the Professional Practice Leader Physiotherapy this position supports the interdisciplinary Hope to Health Team (which also includes social workers nursing psychologists physicians pharmacists etc. in achieving program and patient goals.

This includes providing evidence based clinical assessment treatment and education grounded in physiotherapy theory and relevant postgraduate studies to outpatients with movement dysfunction musculoskeletal and chronic/complex pain issues within a group setting and/or on an individual basis. Provides leadership and expertise in the evaluation and treatment of physical functioning/abilities provides a therapeutic environment and selfmanagement education monitors patient outcomes and adjusts care plans as required by working collaboratively with patients families and caregivers and outside agencies.

What You Bring

Education and Experience:

  • Baccalaureate Degree in Physiotherapy. Three 3 years recent related experience working in a chronic disease management setting including one year working with the specialized needs of patients with complex pain or an equivalent combination of experience education and training.
  • Current registration with the College of Physical Therapists of B.C. Eligible for membership with the Canadian Physiotherapy Association.

Skills and Abilities:

  • Knowledge of physiotherapy clinical practice related to treatment and care of patients with chronic/complex pain conditions.
  • Knowledge and ability to use relevant evidencebased approach in pain management and musculoskeletal assessment and treatment.
  • Comprehensive knowledge of the pathophysiology of pain.
  • Ability to apply critical thinking and evidence based approaches to develop assessment treatment and discharge plans.
  • Demonstrated ability to work collaboratively as a member of an interdisciplinary team and to develop strong working relationships with patients families other health care providers as well as outside stakeholders and community therapists.
  • Ability to develop review evaluate and write clinical protocols and guidelines and patient education materials.
  • Knowledge of outcome measures related to pain management also research methodology and processes.
  • Demonstrated ability to teach and to conduct and facilitate formal and informal presentations demonstrations and lectures related to physiotherapy and complex pain management.
  • Demonstrated ability to facilitate therapeutic and educational patient groups.
  • Demonstrated ability to deal with and/or guide others in conflict resolution issues.
  • Physical emotional and cognitive ability to carry out the duties of the position.
  • Demonstrated ability to work independently and prioritize issues and workload.
  • Demonstrated ability to maintain accurate progress notes discharge summaries and compose medical legal reports.

About Us

The Hope to Health Clinic Research and Innovation Centre in the Downtown Eastside is a unique and innovative health clinic comprised of a multidisciplinary clinical team focused on providing primary and specialty care for patients/ clients in the Downtown Eastside(DTES). The R&I centre is staffed by an interdisciplinary team (e.g. physicians nurses counsellors peer navigators) and provides wraparound healthcare to clients ensuring they receive the full continuum of care under one roof. Hope to Health Clinic works as part of the BC Centre for Excellence in HIV/AIDS.(BCCfE).

The BCCfE is a worldrenowned HIV/AIDS Centre with innovative low barrier approach to healthcare delivery in clinical practice and an integrated group of research concentrations in Laboratory Sciences Clinical Trials Population Health and Epidemiology Health Economics and Professional Education Programs. A multidisciplinary team of clinicians including Physicians Nurses Social Workers and Peers and researchers including Health Economists Epidemiologists Clinical Researchers Statisticians Programmers and Data Analysts work collaboratively to improve the health of British Columbians with HIV and communities facing socioeconomic barriers in accessing healthcare.

Your Day to Day

  • Develops plans and implements advanced physiotherapy assessments and treatments for patients with complex psychosocial and complex pain conditions in a coordinated approach and in accordance with established standards of practice and program standards by methods such as identifying patient issues/needs planning implementing evaluating and documenting clinical services. Collaborates with other members of the interdisciplinary team the patient family and caregivers and outside stakeholders in the creation of an appropriate ongoing care plan.
  • Develops and delivers learning resources for health care team members and for patients and their families/caregivers based on best practices regarding the management of the functional and physical effects of living with complex pain by methods such as researching and reviewing professional literature consulting with other professionals and reviewing and modifying education programs.
  • Collaborates with other members of the interprofessional team to develop and plan evidence based programs services and resources required by patients with chronic/complex pain. Acts as a clinical resource in meetings: provides expertise input and recommendations to the team with the goal of enhancing patient care service delivery and clinical outcomes in chronic/complex pain management.
  • Participates and/or leads continuous quality improvement activities by methods such as evaluating clinical activities and services selecting and utilizing outcome measures to identify and troubleshoot inconsistencies evaluate effectiveness of changes in patient care practice attending and contributing to interdisciplinary meetings and providing advice as required.
  • Establishes and builds positive and effective working relationships with a variety of stakeholders such as members of the Hope to Health team other health care professionals the patient family members/caregivers and community stakeholders.
  • Provides education in the role of physiotherapy to medical pharmacy nursing psychology rehabilitation assistants occupational therapy and physiotherapy students
  • Maintains and updates own clinical and professional knowledge of current developments in physiotherapy by methods such as reviewing professional literature consulting with other professionals actively participating in ongoing relevant education programs and chronic/complex pain special interest groups.
  • Participates and collaborates on research projects identifies implications of research findings and imbeds new developments into the program procedures and processes used for the patient population with chronic/complex pain.
  • Educates members of the interdisciplinary team on most current information relating to physiotherapy practice and chronic/complex pain management trends and evidence best practices and updates educational material and/or policies to reflect changes.
  • Maintains administrative reporting such as completion of workload measurement information records and statistics as required.
  • Maintains patient records by methods such as documenting patient assessments charting patient progress notes composing letters and reports (including medicallegal reports) in accordance with established standards and policies and procedures to meet regulatory requirements and documenting for future reference.
  • Performs other related duties as required.
