Job Description

For over 117 years Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160plus programs and services for children youth adults seniors and those struggling with mental illness.

The ACT team provides an integrated set of evidencebased treatment rehabilitation case management and support services delivered by a mobile multidisciplinary mental health treatment team. ACT supports client recovery through a highly individualized approach that provides clients with the tools to obtain and maintain housing employment relationships and relief from symptoms and medication side effects. ACT services are developed through the personcentered service planning process and adjusted through the process of daily team meetings.
STATEMENT OF THE JOB:

The Psychiatrist provides psychiatric services and consultation. In conjunction with the Clinical Director/Team Leader the Psychiatrist has overall clinical responsibility for monitoring client treatment and staff delivery of clinical services. The Psychiatrist provides psychiatric and medical assessment and treatment; clinical supervision education and training of the team; and development maintenance and supervision of medication administration and psychiatric and medical treatment and procedures. The Psychiatrist provides clinical leadership to the ACT team staff responsible for delivering intensive inhome and communitybased services to individuals living with severe mental illness whom have a treatment history characterized by frequent use of psychiatric hospitalization and emergency rooms involvement with the criminal justice system alcohol/substance abuse and lack of engagement in traditional outpatient services. The Psychiatrist under the direction of the Clinical Director is responsible for ensuring services provided by the ACT team include a full range of clinical treatment psychosocial rehabilitation and community support services designed to promote recovery by improving psychiatric symptoms preventing relapse teaching skills providing direct assistance and securing community resources necessary for successful functioning in work school home and social relationships. The Psychiatrist is responsible for providing treatment to individuals within their community especially to those who choose not to present to the office as needed and in partnership with other medical treatment staff on the ACT team. The role will also require 24/7 oncall consultation and a willingness to be flexible.

* Provide psychiatric and medical assessment and treatment to clients served by ACT team.
* Serve as the medical leader for the team overseeing team delivery of clinical services participating in team meetings to evaluate risk and risk management needs.
* Prescribe medications (utilizing eprescribing function of EHR) and monitor client response to prescribed medications.
* Coordinate client access to medications with the PNP RN and LPN nurses including administering injectable medications when needed.
* Coordinate the delivery of the psychiatric care when sharing the caseload with a psychiatric nurse practitioner.
* Monitor treatment and provide follow up visits treatment for cooccurring substance use disorders and respond to crisis situations.
* Promote and deliver evidencebased integrated treatment for clients with cooccurring substance use disorders.
* Documentation requirements include writing progress notes for all facetoface and telephone contacts with clients and collaterals completing treatment plans as needed & writing thorough and clinically accurate mental status assessments.
* Provide 24/7 oncall crisis response consultation.


* Doctor of Medicine (M. D. Doctor of Osteopathy (D. O. or the equivalent required.
* Must be currently licensed as a physician by the NYS Education Department and certified by or be eligible to be certified by the American Board of Psychiatry and Neurology.
* Board Eligible or Board Certified in Psychiatry required.
* Valid New York State DEA number required.
* Apply for and maintain valid NPI number
* One year of postgraduate hospital training in an accredited residency program approved by the Accreditation Council for Graduate Medical Education the American Osteopathic Association or the Royal College of Physicians and Surgeons of Canada required.
* Eligible for full and unconditional participation in the Medicaid and Medicare programs
* Experience in community psychiatry working with people with serious mental health conditions cooccurring substance use disorders and homelessness.
* Bilingual Spanish/English speaking preferred.
* Able to lift up to 10 pounds.
* Able to travel to multiple locations as needed.
