Role Responsibilities:
- Coordinate both formal and informal supports
- Manage clients with complex needs and handle crisis situations
- Collaborate with a Multidisciplinary Panel
- Engage with Mental Health Justice System Housing and Health sectors
- Undertake clerical administration and report writing
- Network and communicate with key stakeholders
- Work independently either from home or onsite.
- Work in a sensitive complex and demanding environment with a high degree of autonomy and selfmanagement.
Qualifications & Experience:
- Relevant qualification in the Human Service Sector or a minimum of 3 years experience in a similar disability sector role
- Preferred: Degree in Social Work Psychology Occupational Therapy Counselling Human Services or Disability Studies
- Experience in Forensic Disability or Forensic Mental Health is a plus
- Demonstrated knowledge of the NDIS and experience working with its participants
- Valid Drivers License with a suitable insured vehicle
- A current Working with Vulnerable People registration
- Recent police check (or willingness to obtain one).
