Health Solutions is a premier wellness center focused on wholeperson care. With over 400 employees in Southern Colorado you would be joining a mighty team of support and administrative staff case managers clinicians physicians nurses and others in our efforts to improve the health and wellbeing of our community.
Were looking for a Clinical Care Specialist to join our Adult Outpatient team.
What Youll Be Doing
Specifically the Clinical Care Specialist:
- Makes appropriate referrals to address mental health and substance use needs
- Provides behavioral health counseling group treatment and resourcebased case management
- Collaborates with other treatment providers to ensure a teambased approach to treatment when applicable
- Identifies coping mechanisms and natural supports that may be available
- Teaches and enhances client coping symptom management problemsolving independent living and socialization skills
- Develops implements and reviews treatment plans with clients
- Maintains competency and proficiency with our electronic health record system
- Performs other jobrelated duties assigned by the director or his/her designee

Physical requirements include the following:
- Frequently stays stationary for prolonged periods of time.
- Frequently moves within and between facilities.
- Frequently exposed tostressful oremergentsituations.
- Frequently operatesa computer and other office equipment to record health data.
- Constantly communicates with clients and others.
- Occasionally moves equipment and/or materials up to 20 pounds.

Qualifications
What Were Looking ForThe MustHaves
- Bachelors degree in social work psychology criminal justice or a closely related field
- experience in case management/community systems navigation
- experience working with persons diagnosed with severe & persistent mental illness and substance use disorders
- exceptional flexibility time management and documentation skills
- Colorado drivers license a good driving record and access to transportation during work hours.
