Job Description

Overview

The Dartmouth Cancer Center at DartmouthHitchcock Medical Center and New London Hospital seek a Nurse Practitioner to work collaboratively to provide high quality compassionate evidencebased patientcentered care through the Section of Oncology in our oncology clinic at New London Hospital.

This is a full time benefited salaried position. This role assesses and manages the healthcare needs of hematology and medical oncology patients emphasizing health promotion and disease prevention as well as the diagnosis and treatment of acute and chronic illness. This individual functions as part of the provider team to care for the departments panel of patients including hematology and medical oncology patients as well as medical infusion patients.

We work in a collaborative environment where treatments are developed for patients in coordination with local health care facilities such as New London Hospital or Dartmouth Cancer Center at DartmouthHitchcock Medical Center in Lebanon and in conjunction with the referring physician patient and family. We are looking for a Nurse Practitioner to join our team caring for patients in their local community including oncology and medical infusions.

Its research clinical care and training are integrated at the Cancer Center with priority on translation of innovation interdisciplinary care team science and career development.

About New London NH:

Founded in 1918 New London Hospital is a notforprofit Critical Access Hospital serving 15 communities within the Kearsarge/Lake Sunapee region in central New Hampshire providing primary care and specialized clinical services in a patient and family centered environment.

As a strategic partner and member of Dartmouth Health NLH is able to bring healthcare experts into the community both in New London and at our Newport Health Center (NHC) so residents are not required to travel away from their home community to seek academiclevel care.

Responsibilities

  • Utilizes clinical skills in the evaluation diagnosis and treatment of patients with cancer or hematologic disorders.
  • Consults and collaborates with a department physician or appropriate specialist to plan and manage the care for patients with unstable complex acute or chronic illness.
  • Ensures exemplary care and facilitates an environment that is centered on the needs of patients and their families and support for the nursing staff
  • Manages therapeutic regimens for acute and chronic conditions associated with the care and treatment for cancer patients or patients with hematologic disorders
  • Monitors toxicities and assesses clinical responses. Implements a revised plan of care as needed.
  • Provides education for patients receiving chemotherapy/immunotherapy in the Infusion Suite.
  • Participates as a member of the interdisciplinary team to provide longitudinal care for hematology/oncology patients and episodic care for unscheduled urgent/emergent needs of those patients receiving care in the Infusion suite throughout various stages of treatment.
  • Serves as a clinical authority and resource providing education and consultation to healthcare team members.
  • Provides guidance and psychological and emotional support to patients and families
  • Performs problem focusedhealth history physical and psychosocial assessment and prescribes appropriate evidencedbased treatments.
  • Addends and modifies chemotherapy/immunotherapy orders as part of each patients treatment regimen.
  • Orders appropriate laboratory diagnostic and radiological tests to evaluate patients current health status.
  • Prescribes medications medical equipment devices physical and occupational therapy and home health services as needed.
  • Actively participates with the Advanced Practice Provider team of the Dartmouth Cancer Center through attendance at monthly team meetings annual retreat journal club biannual peer review process
  • Other duties as requested.

Required Licensure/Certifications

  • Graduate of an accredited APRN program.
  • Board certified by the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
  • Eligible for APRN licensure in the State of New Hampshire and the State of Vermont.
  • Eligible for Federal DEA Registration.
